A 51-year-old man treated for 2 years with flupentixol, an incisive neuroleptic,
developed severe dyspnea due to intermittent, rhythmic, and dystonic movements
of the vocal cords and upper airway. Local injections of botulinum toxin
resulted in spectacular regression of laryngeal spasms and major improvement
in breathing. This case emphasizes the risk of upper respiratory dyskinesias
associated with neuroleptic treatment and shows the feasibility of a new
local treatment in this life-threatening disorder."

We report
a patient with proximal myotonic myopathy who was treated with neuroleptics
because of exacerbating schizophrenia. Under therapy with fluanxol, the patient
developed muscle stiffness and oculogyric cramps. Treatment with both amisulpride
and olanzapine lead to markedly elevated serum creatine kinase levels. An
in-vitro contracture test was positive for halothane. Thus, in patients with
all kinds of multisystemic myotonic myopathies, a susceptibility for malignant
hyperthermia and intolerance towards neuroleptics should be taken into
account."

El
síndrome neuroléptico maligno es una complicación rara
aunque muy grave que puede ocurrir con la administración de
neurolépticos.
Sus
manifestaciones incluyen: hipertermia, espasmos musculares alteración
de la conciencia, presión sanguínea lábil, taquicardia,
diaforesis.
Más
probable sería una manifestación de una reacción adversa
de tipo extrapiramidal (acatisia, distonía, discinesia tardía,...)
con estos fármacos, distonía en este caso. Se manifiesta por
contracciones musculares involuntarias, sostenidas y repetidas, que causan
movimientos espasmódicos.
En enfermos
de Parkinson es frecuente.
La distonía
es relativamente frecuente con el uso de neurolépticos (probablemente
debido a su mecanismo de acción bloqueadora de los receptores
dopaminérgicos D1 y D2). La asociación de un antidepresivo
tricíclico (melitraceno) y de un antipsicótico (flupentixol),
que forman parte del "Deanxit cp.", sin duda van a causar algún
'torbellino' en el SNC. No estoy seguro, pero creo que esta asociación
tan solo está registrada en España (no lo está en
ningún otro país de la UE).
