RESUMEN

Describimos un caso de meningitis por L. monocytogenes en una persona de edad avanzada sin otra causa de inmunodeficiencia. El cuadro debuta con fiebre, cefalea, naúseas y vómitos. Unos días más tarde sufre un empeoramiento de estos síntomas además de desarrollar un síndrome confusional. El TAC craneal y el electroencefalograma no contribuyeron al diagnóstico del proceso; así pues, éste se confirmó por el aislamiento del microoganismo en el LCR y la aglutinación indirecta, que mostró altas concentraciones de anticuerpos específicos, siendo negativos los hemocultivos y sin presentar previamente evidencia clínica de sepsis. El tratamiento combinado de ampicilina y gentamicina dio un buen resultado, evolucionando el paciente favorablemente en unos días.

ABSTRACT

We present one case of meningitis by L.monocytogenes in an elderly person without another cause of inmunodeficiency. He developed a syndrome characterised by fever, headache, nausea and vomiting. Several days later these symptoms got worse and he developed a severe brain stem dysfunction. Neuroimaging techniques were normal and they did not positively contribute to the diagnostic process, which was confirmed by the isolation of the microorganism in CSF and indirect agglutination , which showed high concentrations of antibodies, although blood cultures were negative without having previously presented clinical evidence of sepsis. The use of a specific antibiotic therapy led to a favourable clinical outcome in few days.
